Instructions: U.S. Postal Service Form 1583
The USPS form 1583 must be filed with any of Mail Receiving Agencies you choose. These services are regulated by the U.S. Postal Service, and your Mail Receiving Agency is required to provide a copy of your Form 1583 to the Postal Service. You should make sure the service you use is in compliance with these rules and regulations, including all of the identification and notarization rules.

Form 1583 Instructions
- Residential/Personal Use: For personal mail
- Business/Organization Use: Only if you are receiving mail for a company
- Your full name
- Your phone number
- Your Email Address
- Your home address in your country (Must match your address document)
- Your name (as shown on ID)
- ID number
- Issuing government
- Expiration Date
- Check the ID type (most international customers check Passport)
- Utility bill
- Bank statement
- Phone/internet bill
- Lease agreement
- Insurance policy
- Vehicle registration (Driver license can be used for photo OR address, but not both)
- Your Name
- Home address
- Select the type of address you are using
Because the service is required to have the originals, send the completed form, along with the copies of your identification to your mail forwarding service.
