Formulaire 1583 Instructions

1. Private Mailbox (PMB) Information
Add date Shipito suite was opened.
2. CMRA (Mail Forwarder) Information
This is your Shipito address including your suite number.
3. Type of Service Requested
  • Residential/Personal Use: For personal mail
  • Business/Organization Use: Only if you are receiving mail for a company
4. Applicant Information (YOU)
Fill out:
  • Your full name
  • Your phone number
  • Votre Adresse E-mail
  • Your home address in your country (Must match your address document)
5. Authorized Individual (Optional)
Only fill this out if you want another person to pick up your mail inside the U.S. Most international customers leave this section blank.
6. Transferring Mail to Another Address
Because you are using a mail forwarding company, USPS requires you to confirm the address where the mail will ultimately go.
7. Business/Organization Information (ONLY for business users)
Fill this out only if you marked "Business Use" in Section 3.
If personal customer, leave entire Section 7 blank.
8. Photo ID Information (Applicant)
Enter your passport or other photo ID details:
  • Your name (as shown on ID)
  • ID number
  • Issuing government
  • Date d'expiration
  • Check the ID type (most international customers check Passport)
9. Address ID Information (Applicant)
This must show your home address (same as Section 4).
Choose one:
  • Utility bill
  • Bank statement
  • Phone/internet bill
  • Lease agreement
  • Insurance policy
  • Vehicle registration (Driver license can be used for photo OR address, but not both)
Enter:
  • Votre nom
  • Home address
  • Select the type of address you are using
10. & 11. Authorized Individual ID (Only if Section 5 has someone listed)
If you did not list an authorized person, leave Sections 10 and 11 blank.
12. Additional Recipients of Mail
If you want additional adults to legally receive mail for your suite use this section to list their names. Each adult listed must be able to provide two forms of ID.
13. Your Signature (Applicant)
Sign and date the form.
Final Steps
Attach the photo IDs used for verification during notarization
Get the form notarized (online notarization is OK).
Mail form if notarized in person or upload and send digital form to Shipito if e-notarized.
